REFERRAL & ADMISSION

VIBRA Specialty Hospital of Portland is a 100% referral based facility. This means that every patient who is transferred into our hospital has been referred from a physician, case manager, insurance company or family member whose loved one is at another facility. Facilities from which a patient can be referred include:
  • Short term Acute Care Hospitals-ICU/Telemetry/Medical-Surgical units
  • Skilled nursing facilities
  • Aquatic therapy
  • Emergency room
  • Acute rehabilitation facilities
  • Traditional care units
  • Foster homes
  • Physician offices
  • Home
  • Insurance company case managers

A referral is made by placing a call to the VIBRA admissions office (503-257-5667) to request a patient evaluation
  • A Clinical Liaison completes an on-site evaluation and verifies insurance eligibility (usually the same day)
  • A team of clinicians inside VIBRA, including a physician, determine appropriateness for LTACH admission and approves patient for transfer
  • Equipment and service needs are determined
  • Authorization from insurance company is obtained
  • The Clinical Liaison meets with the patient and/or their family to discuss information about VIBRA, answer questions and invite them for a tour of the facility
  • The Clinical Liaison works with the referring facility to facilitate transfer to VIBRA Specialty Hospital of Portland

Case Management
Upon admission, a case manager is assigned to every patient. She serves as the liaison between the patient and the physician, nursing staff, and therapists. She is a major part of the inter-disciplinary team. She organizes patient care conferences with the medical staff, which patients and their families are encouraged to attend. The case manager plays very important role in the continuum of care for the patient’s entire stay right into their discharge.
We accept Medicare, Private insurance, and most managed care plans.


WHAT TO EXPECT WHEN A PATIENT ENTERS VIBRA SPECIALTY HOSPITAL OF PORTLAND
Upon admission to VIBRA Specialty Hospital of Portland, each patient is assigned an inter-disciplinary team, directed by a physician, that develops a personalized care plan for that individual patient. Each member of the team works with the patient, family members, and each other to carefully monitor the patient’s care and progress. A patient’s length of stay and discharge disposition is determined by the progress that is made while in the LTACH setting.
